Creating an Impact Questionnaire from a Template

You can create an impact questionnaire in change orders and change requests to collect additional information about the proposed changes. You can respond to, then complete impact questionnaires to provide additional information about the impact of the regulatory request.

Required access roles:

  • Users with the Change Administrator, Change Coordinator, or change request owner access roles create an impact questionnaire when the change request is in the Evaluate state.
  • Users with the Change Administrator, Change Coordinator, or change order owner access roles create an impact questionnaire when the change order is in the Prepare or In Work states.


Before you begin: View the Impact Questionnaires tab in a change order or change request.
  1. In the page toolbar, click Create Impact Questionnaire from Template .
  2. Enter these details:

    Column NameDescription
    Name To enter a custom name, clear the AutoName check box. Otherwise, leave the box checked to allow the system to generate the name.
    Impact Questionnaire TemplateSelect the impact questionnaire template that you want to use to create the impact questionnaire.
    DescriptionType a detailed description of the impact questionnaire.
    Due DateSelect the date by which the impact questionnaire should be completed.

  3. Click OK.

The new impact questionnaire is added to the change order or change request.
