Creating an Impact Questionnaire Template

Change Administrators and Document Center Administrators can create an impact questionnaire template that is used to create impact questionnaires in change orders and change requests to collect additional information about the proposed changes. Regulatory Request Administrators and Regulatory Administrators can create an impact questionnaire for a regulatory request type that identifies the appropriate reviewers based on responses to the questions.


Before you begin: View the Templates tab for impact questionnaires. For more information, see Template Tab for Impact Questionnaires.
  1. In the page toolbar, click Actions > Create New .
  2. Enter these details:

    Column NameDescription
    Name To enter a custom name, clear the AutoName check box. Otherwise, leave the box checked to allow the system to generate the name.
    DescriptionType a detailed description of the impact questionnaire.
    RequirementSelect whether the impact questionnaire must be completed:
    • Mandatory
    • Optional
    Responsible OrganizationSearch for and select the organization responsible for the impact questionnaire template.
    Responsible RoleSearch for and select the role responsible for completing the impact questionnaire template.
    Note: By default, both the Type and Policy are Impact Questionnaire Template.

  3. Click OK.

You must configure the impact questionnaire questions and add any attribute properties that you want to be part of the impact questionnaire before you make the impact questionnaire active.
