Creating a Questionnaire

Users with the appropriate role in an app can define a questionnaire for a template. The questions determine whether sub-questions should be displayed in the questionnaire or related objects should be automatically added when new objects are created from the template.

Change Administrators can create a change questionnaire in a change template that determines whether approval routes, member lists (multiple approvers), people (individual approvers), or impact questionnaires should be added to a change order created from that template.

Regulatory Request Administrators can create a reviewer questionnaire to select the appropriate reviewers for all regulatory requests of that type.


Before you begin: Do the following before creating a questionnaire:
  • Determine the structure for the questionnaire. See Designing Questionnaires.
  • Create and activate all related objects to be associated with questions in change questionnaires. For example, to add an impact questionnaire to a change order or change request as a result of a question response, the impact questionnaire must already exist and be in the Active state.
  • View the Questionnaire page tab for the template.
  • Demote the template to the Inactive state to edit it.
  1. On the page toolbar, click Enable Edit .
  2. Click Insert new row to add a new question.
    The app adds a row to the table.
  3. Enter these details:

    Column NameDescription
    QuestionEnter the text of the question.
    CategorySelect the question category:

    DecisionA question that has one of three responses:

    Yes

    No

    Unknown

    DescriptiveA question that has a free-form text answer.

    ResponseThe response that will result in a specific related object being added to the objects created from the template. Only sub-questions and assigned objects have a response associated with them.

  4. To create additional questions (not sub-questions), make sure no questions are selected, then repeat steps 2 and 3.
  5. Click Save to save the questions before creating any sub-questions.
  6. To create a sub-question:
    1. Select the question under which you want to create a sub-question.
    2. Click .
      The app adds a row indented beneath the selected question.
    3. Repeat Step 3.
  7. Add as many questions and sub-questions as needed to define the questionnaire.
  8. Click Save to save all questions and sub-questions before assigning any related objects.
  9. For change questionnaires, assign the related objects to question responses:
    1. Select the question or sub-question.
    2. From the page toolbar, select Actions > Related > Assign.
      The Search Refinements page opens and restricts your search to the types of related objects appropriate for that type of questionnaire. When creating a change questionnaire, you can select route templates, individual people, member lists, or impact questionnaires.
    3. Enter your search criteria, then select the all of the objects that you want to assign to the question.
    4. In the Parent Response column for each selected object, select the question response that you want to associate with this assigned object.
    5. Click Submit.
  10. If necessary, select Actions > Copy Existing then search for and add existing questions to the questionnaire.
  11. If necessary, change the sequence of the questions and sub-questions using the Move Up and Move Down commands.
  12. Click Save.
  13. Click Disable Edit.
