Questionnaire Tab for Reviewer Questionnaire

The Questionnaire tab for regulatory request types allows the Regulatory Request Administrator to define a question and response structure, then assign sub-questions and reviewers to specific question responses so that the appropriate reviewers are assigned to all regulatory requests of that type.

To access the Questionnaire tab for regulatory request types:

  1. Log in to the Market Registration app with the Regulatory Request Administrator role.
  2. In the navigation pane, click Regulatory Request Configuration.
  3. Select the regulatory request type.
  4. In the navigation pane, click Reviewer Questionnaire.
  5. Select the Questionnaire page tab.

Columns

Column NameDescription
QuestionThe text of the question or the name of the related object. The top row shows the name of the parent item. The remaining rows show questions, sub-questions, and related objects. You can expand a question to view the related objects, such as route or person, assigned to it.

Click the question text or related object name to open the object.

CategoryThe question category:

CategoryDescription
DecisionA question that has one of three responses:
  • Yes
  • No
  • Unknown
DescriptiveA question that has a free-form text answer.
Each Decision response can be associated with one or more related objects.

ResponseUsed for rows other than parent questions. If the answer to the parent question matches the response on this line, then the assigned related object is added to objects created from the template, or the assigned sub-question is displayed.
DescriptionThe description of the object.
TypeThe type of object on that row of the questionnaire, such as question, route, or person.
StateThe current lifecycle state of the object. All of the objects associated with a questionnaire are either Active or Inactive.

Actions/Toolbar Commands

These actions are only available when the template is in the Inactive state.

Actions > Import Imports questions from a .csv file.-

Actions > Copy Existing Copies an existing question into the list. If the question has sub-questions, the entire question hierarchy is copied into this list.

You can change the response value (Yes or No) for each question in the search results, or after submitting your selections.

Actions > Delete Deletes the selected questions. Click OK to confirm.-
Actions > Active/Inactive Toggles the selected questions between the Active and Inactive states.

A question must be in the Active state when an object is created from the template for it to be added to the new object.

This action only works with questions. It does not work with other items.

Actions > Related > AssignAssigns a related object to the question. The types of related objects that you can assign to a question depends on the type of questionnaire.

The search results contain only valid objects that can be assigned to the question.

In the search results, select the response for the question that will cause the assigned related objects to be associated with the objects created from the template.

Actions > Related > Disconnect Disconnects the selected related object from the question.-
Move Up Moves the selected question up in the questionnaire's order.

The page must be in Edit mode to use this action.

Move Down Moves the selected question down in the questionnaire's order.

The page must be in Edit mode to use this action.

Enable Edit Toggles the page into Edit mode. In Edit mode, you can add rows for new questions and edit the Question (question text), Description, and Response fields for existing questions.-
Disable Edit Toggles the page into View mode. All information is read-only.-
Insert New Row Inserts a new row in the table so that you can add a question.

The page must be in Edit mode to use this action.

Creating a Questionnaire
Remove Inserted Rows Removes an inserted row that has not yet been saved. If you have already saved your edits, you cannot remove the inserted row, but you can still delete it.

The page must be in Edit mode to use this action.
