Root Causes Page

This page lists the defect causes that are identified and created for the CAPA.

To access this page:

  1. From the CAPA Page, click the name of the required CAPA.
  2. Click the CAPA Details tab.
  3. Click the Root Causes page tab.

Note: On the CAPA page, if you right-click a CAPA name, you can view the primary root cause list and can access the properties page of a defect cause. The top-level root causes are sorted according to the date of creation, with latest on top. If there is a tree of defect causes, only the latest root cause in each branch of the tree is displayed in the list.

Columns

Column Name Description
Name Name of the defect cause.
Type Type of the object.
Title Title of the defect cause.
StateState of the defect cause. Possible states are Identified and Addressed.
PrimaryShows if the defect cause is primary.
FactorClassification of the defect cause.
Sub-FactorSub-classification of the defect cause.
DescriptionDescription of the defect cause.
TaskShows if a task is associated. Click to view its details.
AttachmentObjects attached to the defect cause. Click to view or add attachments.
OwnerName of the defect cause owner.

Filters

Based on your role, you can use the following filters to filter the Root Causes page by states or statuses.

  • Primary: Displays only the primary defect causes.
  • Root: Displays the defect causes that do not have any child defect causes.
  • All: Displays all defect causes.

Change OwnerTransfers the defect cause to the selected person. Click and from the Search Results page that appears, select the required user and click Submit.--
Mass Promote Promotes the selected defect causes to the Addressed state.
Notes:
  • The selected defect causes must be in the Identified state.
  • You must be the owner of the defect cause or the CAPA.
  • All associated tasks must be in the Complete state.

Cancel Cancels the selected defect causes and disconnects all associated tasks. Click OK to confirm.

Delete Deletes the selected defect causes. Click OK to confirm.

Create NewCreates a new defect cause.Creating a Defect Cause
Enable Edit/ Disable EditActivates or deactivates edit mode in which you can modify the Title and Description. If the defect cause is in the Addressed or the Cancelled state, you can modify only the Description. Click Save when done.--
